Permission to Skate Forms
(OMHA PTS for AAA and Junior)

The Ontario Minor Hockey Association (OMHA) requires that every player trying out for a AAA team obtain a Permission to Skate (PTS) form from their home centre prior to stepping on the ice.  In order to receive a PTS form from the Newmarket Minor Hockey Association (NMHA) a player must be a resident of Newmarket.  Please note that a permission to skate form is not a player release nor is it an OHF AAA Waiver form.

The NMHA can only provide a PTS form for the York Simcoe Express (YSE) for players U9 - U13. We cannot provide a PTS form for any other AAA Zone. If a player is cut from the YSE they can request an OHF AAA Waiver form from YSE to allow them to tryout with an adjoining AAA zone.

U14 - U16 may try out with any OMHA AAA team, PTS is required. Player movement to another OHF Member must follow current OHF waiver regulations.

PTS for U18 to attend AAA tryouts issued in accordance with OHF regulations K1-K27. Player movement to another OHF Member must follow current OHF waiver regulations.

For Junior PTS, if player played on a AAA team this past season, request Junior PTS from the AAA association. If player played for their home centre-Newmarket use link below.

At this time we will only be processing the PTS for tryouts that are being held in the Spring (U14 - U16 and for Junior teams). For Tryouts that are being held in the Fall please apply for PTS in August.

To register for a PTS (U14 - U16 AAA or JR) click on the link below:

NOTE:  If you get cut from AAA, to attend NMHA tryouts, you must have been registered with NMHA for the 2021-22 season, and must purchase a NMHA Rep Tryout Passport. If you were not registered with NMHA for the 2021-22 season, contact the office to confirm you are eligible to attend NMHA tryouts.
