2024 NMHA Rep Evaluation Passport Registration (U8-U9-U12B-R-U18)

THIS FORM MUST BE COMPLETED IN FULL. This completed form must be printed & presented at all evaluations with proof of payment. All Players must attend evaluations as per the NMHA Evaluation Policy found under the Rep Hockey tab.

U18BB
The U18 'BB" team is a non-body checking team. Any player who wants to only evaluate at U18 'BB' must purchase an evaluation passport and select the appropriate 'BB' team option which allows the athlete to go directly to the 'BB' evaluation. Players evaluating for 'AA' and 'A' can elect to move directly to 'BB' evaluations at any time (not required to attend any remaining 'AA' or 'A' evaluations) by notifying the coach via email.

Payment Information

Refund requests will incur a $10 charge only for players who have not attended any evaluations. Requests must be emailed to [email protected]. No refund requests will be accepted after Sept 7, 2024.
